Routine Care - the basic level of care provided to patients and families in their residence.
Respite Care - when caregivers need a break for any reason, a 5 night stay in a contracted facility for the patient.
Acute Care - when the level of patient care requires hospitalization for pain control and symptom management.
Continuous Care - when a patient is experiencing a crisis, our team provides around-the-clock care.
Every patient is different and every family has different needs, so we individualize our care plan to each patient. Oasis is a small company which allows us to be flexible, always keeping the patient's quality of life as our primary goal. Here we give you an idea of the basic services we provide, which may differ according to your needs.
Therapy - When appropriate and ordered by the physician, Oasis may provide speech, occupational and physical therapy.
Continuous Time Care - When a patient is experiencing a crisis, or if the patient is actively dying, our team can provide round the clock care for the crisis period. RNs, LPNs, and CNAs rotate shifts to provide this continued care in a patients home or in a facility like a nursing home or assisted living.
Inpatient Care - Patients are being seen so frequently by the hospice team, hospice care usually means fewer trips to the emergency room or the hospital. However, if the nurse or on-call nurse is not able to handle a situation at home, the patient may be sent to the hospital by our care team for symptom management.
Respite Care - Respite care takes place in a skilled nursing facility which provides the patient with round the clock care while a caregiver is on vacation, taking care of their own medical problems, or just getting a break and some relief. Our team arranges for the transport of the patient to and from the facility and pays for the stay. In addition to the staff at the facility, the hospice nurse and hospice team continue to visit the patient. The respite benefit is available for up to five nights several times each year, all according to patient and family needs.
Hospice pays for all medication related to the patients primary diagnosis, including comfort medications for pain, anxiety, and insomnia.
