Primary Care Physician - The Hospice patient keeps their Primary Care Physician who leads the Oasis Team in creating a care plan specific to each patient's medical needs. The nurses stay in close contact with the patient's physician, becoming his/her eyes and ears in the patient's residence. The team will report to the physician and will work with the physician to make any changes needed in the plan of care.
Remember, hospice benefits are covered 100% by Medicare, Medicaid, and most private insurance. There are no out of pocket costs for the following services, supplies or equipment.
Nursing Visits - Our experienced team of Registered Nurses and Licensed Practical Nurses visit each patient once or twice as week, as needed. They assess all aspects of the patient's health and well-being. The nurse will also bring the medications that we provide to the patient and help them organize their medicines for the week ahead. They will assist the caregiver in understanding how to appropriately give medications and use medical equipment.
On-Call Nurse - We also have an RN on call, 24 hours a day, 7 days a week to answer any questions or to come out to the patient's residence. With frequent nurse visits and the 24 on call nurse, patients usually need fewer visits to doctor's offices, emergency rooms, or the hospital. Knowing that a nurse is just a phone call away, takes away the fear and anxiety many caregivers experience.
Aide Visits - Our compassionate Hospice Aides visit the patient up to 5 times each week. They take time visiting with a patient while tending to their hygiene needs. Hospice Aides spend an ample amount of time bathing the patient, and tending to personal hygiene needs. They may also help fix an easy meal or encourage the patient to drink dietary supplements or water. The Aides will make sure that the patient has the supplies they need, which can include incontinence products, hygiene items, and also the dietary supplements, all provided at no cost to the patient.
Social Workers and Volunteers - The social worker checks in with the patient and the family once or twice a month, or more if needed. They are also available on call. They are available to discuss difficult end of life issues and help families make plans. The social worker becomes a great resource to the family, with the goal of increasing the quality of life for the patient and family. She is also there to help the families look into various programs that may benefit the patient or family. We also provide volunteers, as appropriate, to visit with the patient, to make comforting, uplifting and useful gifts for the patient, and sometimes, to sit with the patient while a caregiver is out or even transport a patient to a doctor's appointment.
Chaplain - The team Chaplain provides pastoral are and spiritual counseling to the patient and family. He visits once a month, but can come more or less depending upon the wishes of the patient and family. If the family wants spiritual counseling from a different denomination or faith, our chaplain will make arrangements for that, as well. Even in cases where the patient seems unresponsive, the chaplain can read appropriate verses or say a prayer for the patient, bringing calm and hope into the patient's day.
